Salient features:
Project Name, Custodian, Name of the Client, Name of Consultant, Popular Name of Bridge (if any), Chainage, Structure Number, NH Number, State Name, Date Of Construction, Last Date of Inspection, Name of River Or Crossing Feature, Type Of Super Structure, Type of Foundation, etc. are shortly informed in this point.
Photograph section:
Some boxes are available to attach photographs. Pictures which are showing the defects of the structure should be attached in the boxes.
Structural Data:
The structural datas are overall length of the bridge, Span arrangement, Height of Approach Embankment, Average Skew angle, Whether Navigable, Horizontal Clearance, Vertical Clearance, Design Discharge in Cum/ Sec, Design HFL & LWL, Design Scour Level at Pier (lowest), Design Scour Level at Abutment (lowest), Founding Strata, Water body lowest bed level, Whether the Bridge is in Grade, Road formation level, Ground level (for approach portion), Deck soffit level or lowest level of superstructure, Clear carriageway width, Overall Deck Width (m), Roadway width (including shoulder) for approaches, Safety kerb width, Footpath width (m), width of Median, width of shoulder, is this high level/ submersible /causeway, etc.
General Data:
General datas are corrosion Protection Measures, Bank Protection & Type, Floor Protection & Type, Is the Bridge Located in backwater (Marine) or chemical affected water body?
Approaches condition:
Type of Terrain, Type of Approach, Material of Approach
Approach Geometrics (Straight/Curvilinear etc.), having any approach Span, Pavement surface (Check unevenness settlement, cracking, potholes, etc.)
Side slopes (Check pitched or un-pitched, condition of pitching/Turfing, any signs of slope failure, etc.)
Erosion of Embankment by rain cuts or any other damage to Embankment, Approach slab (Check, settlement, cracks, movement, etc.)
Retaining walls Type, Retaining wall condition (Check subsidence, tilting, condition of weep-holes, guard-stone and railing, etc.)
Silt and Debris (Check Accumulation of silt and debris on submersible approaches in cutting and embankment).
Protection work:
Type of protection work, guide-bund or protection around abutments or spurs, slope pitching, apron, and toe walls, floor Protection works (Condition if impervious floor, flexible apron, curtain walls and indicate nature of damage if any, etc.), reserve store material, etc.
Waterway:
Obstruction in flow and its impact on flow, island formation, Vegetation growth, etc., flow pattern (Check any abnormal change in Meandering inflow and erosion of banks), Maximum Flood Level observed during the year, afflux from U/s and D/s (Check signs of abnormal Afflux from U/s and D/s watermark on piers if any, etc. should be checked to make a proper waterway.
Bridge Foundation:
Type of Bridge foundation, Material, Condition of Foundation (Check Settlement, abnormal Scour, Tilting, if any, Check damage due to impact of floating bodies, boulders.
Substructure:
Type, Material, Condition (Crack, Settlement, Scour, Tilting, Rusting in exposed Steel, Strains and other damages, etc.)
The efficiency of drainage of the backfill behind abutments (Check functioning of weep holes, evidence of moisture on abutment faces, etc.)
For Subways:
Check the condition of side retaining wall like cracking, disintegration, and seepage, if any
Check large excavations done in the road below in the vicinity of flyover or road over bridge of viaduct.
Seepage, vehicle impact, etc. (For sub-ways report seepage, vehicle impact, if any damage to the foundations, etc.)
Bridge Bearings and Bridge Pedestal:
Type, Material, No. per Abutment, No. per Pier.
General condition (Check rusting, cleanliness, seizing of plates silting, accumulations of dirt in case of submersible bridges).
Functioning (Check excessive movement, tilting, jumping off guides.
Superstructure:
Spalling, scaling, honeycombing, disintegration, cracks, exposed reinforcement, rusting and corrosion on exposed steel reinforcements, excessive vibration, excessive deflection, condition of protective coat or paint, etc.
Check the condition of steel members in case of steel structure.
Check the condition of articulation.
For PSC Structure: Check corrosion of reinforcements (sheathing and tendon if visible), Check cracks, if any, around the anchorage zone for prestressed concrete members.
Expansion joints:
Type of Bridge Expansion Joints, condition, Functioning, condityon of sealing materials, check water leakage through the expansion joints.
Wearing coat:
Material, Surface Condition i.e. Cracks/ potholes/ Bulges, spalling, disintegration, etc.
Details of water proofing system, if any.
Drainage spouts and Vent holes:
Check the condition of drainage spouts (clogging, deterioration, and damage, if any)
Check the projection of the spout on the underside of the bridge deck (see whether structural members are being affected).
For subways report about the adequacy of drainage and pumping arrangements etc.
Report absence of Drainage Spouts.
Check choking of drainage holes provided in the bottom booms.
Check choking of vent holes etc.
Handrail, Parapet wall, Crash Barrier:
Material, Condition, check Expansion joint gap in handrail/parapets/Crash barriers to allow expand during thermal variation.
Footpath:
Width of footpath, material, condition, etc.
Utility:
Present (Yes/No), Type of Utility, Report any damage due to the presence of Utility, Any type of encroachment under the bridge, etc.
This format was released in 2016 by the National Highway Authority of India, the Ministry of Road Transport & Highways, and the Government Of India (NHAI, MoRT&H). As a bridge inspector, personally, I have been using this Proforma Of Bridge Inspection Report As Per IRC SP 35 | IRC SP 18 for making the bridge inspection report for 5 years.
